Forms submitted by 11:59 PM on Thursday, December 8, 2016 may entitle the applicant to $10 OFF YOUR NEXT PAYMENT. Limit, 1 discount per family, regardless of number of evaluations. Parents or guardians of multiple children taking lessons at the CAM may fill out one evaluation for all students if doing so seems appropriate. Those taking lessons with multiple teachers may fill in a separate evaluation per instructor.
